If you have health insurance, please tell our secretary when you make an appointment
and bring any necessary forms and identification with you. Southern New Hampshire
accepts most major health insurance plans and participates with many managed
care networks.

Each time you visit a provider's office, please be prepared to show your insurance
card upon arrival. Your card contains important information that is necessary
for the timely processing of your claim. It also contains your co-payment responsibility
and referral authorization requirements. If insurance information is not provided
at the time of your visit, you will be responsible for the full balance.
Please note that the patient is responsible for any co-payment, coinsurance,
deductible, or any other balance that is not covered by his/her insurance company.
For your convenience, we accept cash, personal checks, VISA, and MasterCard. Payment
is expected at the time of the service.
If you have a balance due after insurance payment is received, you will receive
a monthly account statement. Payment in full is expected within 30 days of
the account statement.
If your insurance plan requires a referral for you to see a specialist, you
must make arrangements with your primary care provider to issue a referral.
Please allow 7-10 working days to complete the necessary paperwork. If your
insurance company has not authorized the referral at the time of your visit,
you will be asked to sign a waiver. This waiver indicates that you accept responsibility
for payment.
