Southern New Hampshire Internal Medicine Associates (SNHIMA)

is a group medical practice providing quality adult Internal Medicine and Internal Medicine subspecialty services. We are one of the largest physician-owned, independent primary care practices in New Hampshire. Patient demand and local population growth fueled the progressive expansion of the practice from a three person group to its present size.

The groups Shareholders and Board Members of SNHIMA live and practice in the great state of New Hampshire. Patient demand and local population growth fueled the progressive expansion of the practice from a three-person group to its present size

The group was founded in 1984 by Douglas M. Eddy, M.D. who started a solo practice at the then new Parkland Medical Center in Derry. In 1993, the practice merged with the late Dr. Edward Yourtee, an independent internist and infectious disease specialist in nearby Windham, New Hampshire.

The desire to integrate physician-owned patient care services in one location led the practice to become a major partner in the construction of Overlook Medical Park, which opened in Derry in 2004. Multiple medical specialties are represented in the facility, including separate practices in gastroenterology, neurology, general surgery, cardiology, urology, OB/GYN, podiatry, physical therapy, laboratory services and hematology/oncology. This concept offers patients the convenience of high quality and cost-effective services under one roof. It proved very popular and further promoted the growth of the practice.

To accommodate the growth of Windham and the adjacent communities of southern New Hampshire, the group opened a Windham office on Route 111 in 2012. This includes an imaging facility and medical specialty offices, duplicating the successful concept in Derry.

In 2016, the group signed two physicians previously employed by the Elliot Hospital and opened a new practice site in Manchester. In 2017, this site moved into a new facility on South River Road in Bedford, with imaging, lab, and medical specialty offices like our other locations.

The practice has been repeatedly recognized by our multiple health insurance partners for delivering high-quality medical care to our patient population, with lower hospital re-admission rates, better management of chronic conditions, and a personal relationship with each and every patient.

This facility includes 13,500 SF of space for SNHIMA, as well as a shared diagnostic imaging center (including American College of Radiology accredited CT, MRI, nuclear medicine, ultrasound, digital mammography, and bone density). Multiple medical specialties are represented in the facility, including separate practices in gastroenterology, neurology, general surgery, cardiology, urology, OB/GYN, podiatry, physical therapy, laboratory services and hematology/oncology. This concept offers patients the convenience of high quality and cost effective services under one roof. It proved very popular and further promoted the growth of the practice.

In 2012, to manage health information effectively and comply with federal initiatives, the practice transitioned from paper to electronic health records, requiring a substantial capital investment in software, hardware and staff. The software and hardware were updated in 2015 to give a more robust platform for data analytics.

In 2016, we signed two physicians previously employed by the Elliot Hospital and opened a new practice site in Manchester. In 2017, this site moved into a new facility on South River Road in Bedford, with imaging, lab and medical specialty offices similar to our other locations.

In 2016, we entered into an affiliation and services agreement with VillageMD, a national company, to enhance patient services and clinical performance.

The group is highly unique in the present health care environment in that it remains independent and physician-owned. The physician shareholders are actively involved in all aspects of the management of the organization.

The practice has been repeatedly recognized by our multiple health insurance partners for delivering high-quality medical care to a sicker than average patient population, with lower hospital re-admission rates, at a lower than average total cost of care.

Strategic Partnerships

SNHIMA works closely with Derry Medical Center, another local independent primary care group, to share certain back office (medical service organization) support services such as billing, enabling economies of scale. Together with Derry Medical Center, we are members of Southern New Hampshire Primary Care IPA, in which we jointly hold payment contracts with certain health insurance payers. Combining resources has allowed us to leverage the sizeable patient panels of both practices to negotiate more favorable reimbursement rates than could be obtained otherwise.

In 2016, we joined in a partnership with VillageMD, an outside company with the stated goal of enabling primary care providers to have a greater focus on proactive care and allowing additional resources to be directed to patients needing extra support outside of the office visit, with the goal of improved clinical outcomes and financial success in a value-based care payment environment. We participate in a Medicare Shared Savings Program through the VillageMD New Hampshire Accountable Care Organization, approved by the Centers for Medicare and Medicaid Services.
